University of Washington Medical Center has an outstanding opportunity for a part-time (65% FTE, 26 hours/week), day shift, SOCIAL WORK ASSISTANT 2.
This position serves as a member of the University of Washington Social Work and Care Coordination department, primarily assisting the Social Workers with the coordination of the treatment and discharge planning process for patients.
SCHEDULE This position will work business hours on weekdays. Monday & Tuesday are confirmed as 8 hour days. The remaining days of the week are to be determined.
Assist in arranging for Skilled Nursing and other post acute care placements post-discharge, based on information obtained from clinical staff
Assist in arranging home services, such as home health, home infusion, wound vacs,or in-home caregivers
Assist in arranging practical needs and emergency assistance, such as transportation, parking, food or local lodging. Demonstrate problem solving skills in working with resources outside the medical center
Assist the social worker and the patient/family in locating non-medical housing (respite, shelter)
Research and update community resource information as needed
Assist patients and families in accessing educational resources relating to the medical and psychosocial impacts of their illness
Participate in the training of Social Work Students and new employees
Participate in practicum instruction by developing training manuals, online materials
Document interventions in electronic medical record
REQUIREMENTS Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent. OR an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.
