Hospice care involves support for patients and their loved ones. It is not so much a place, but rather an approach to health care emphasizing symptom management while helping patients and families to cope with psychosocial and spiritual issues. UC Davis Hospice serves patients of all ages, including newborns and children, as well as adults. The Home Care Services Department comprises the Home Health, Hospice, and Palliative Care Programs. Patient conditions include acute and chronic conditions spanning across many care specialty areas and age ranges. This position is responsible for the care of hospice patients in the home and occasionally inpatient setting. May also be required to float to other areas of Home Care including Home Health and Home Based Palliative Care. Provides direct patient care to a diversified patient population. Applies knowledge, experience and judgment to determine the importance of a situation, set priorities, and use abstract thought to evaluate clinical situations.
