This is a new clinic to UC Davis Health that will promote healthy aging through an integrated approach across the lifespan and all care settings. As the clinic grows, we plan to have the following services within our clinic: Geriatric Medicine, Neurology, Cognitive testing with Neuropsychology, Geriatric Psychiatrist, Family Caregiving Institute and several other patient services. The Healthy Aging Clinic will be a part of providing age-friendly care, supporting family caregivers, leveraging cutting-edge research, integrating technology for independent living, educating the next generation of clinicians through degree programs, fellowships and certifications. Responsibilities Patient Navigator Clinical 4, within the Midtown Healthy Aging Department, serves to improve the health of its patients through empowerment, self-management support, care coordination, and education. Technical leader with a high degree of knowledge in the overall field and recognized expertise in specific areas; problem-solving frequently requires analysis of unique issues / problems without precedent and / or structure. May manage programs that include formulating strategies and administering policies, processes, and resources; functions with a high degree of autonomy. Conducts case management and coordinates care for UCDH patients through telephonic communication with individual patients on an ongoing, scheduled, and as needed frequency. The department aims to develop an effective and cost-efficient system of delivering and managing patient-centered, quality care for those patients with chronic illness, patients that present at risk of illness, and those patients with an acute, chronic and/or complex situation. The Patient Navigator Clinical 4 will work independently, but receives direction concerning the overall goals and objectives of the department and participates in actions to achieve these departmental goals. Utilizes specialized knowledge and skills within major areas of focus, such as patient / family, nurses and nursing practice, and the organization / system. Provides specialized clinical education in regard to treatment and liaisons with the patient's clinical service providers to address specific concerns. Has in-depth specialty expertise and focus.
