As an Internal Medicine Clinical Pharmacist Specialist, you will be responsible for developing and maintaining a high-level clinical pharmacy practice in adult Internal Medicine. Specialist will round with an Academic Medical Teach Service participating in multidisciplinary rounds and geographical medical/surgical patient care coverage at Banner University Medical Center – Phoenix. Patient care responsibilities include ensuring appropriate therapy for rounding service and completing and managing pharmacokinetic, anticoagulation, and pain consults. You will be responsible for developing, implementing, and sustaining clinical pharmacotherapy programs that enhance patient care and medication safety. Expected to participate in educational initiatives and staff development; develop and provide oversight for the quality assurance of clinical pharmacy programs that improve patient care and experience; and participate on system and hospital committees and pharmacy and therapeutics committee as assigned. This is a day shift position scheduled to work Monday-Friday with rotating on-call and weekend coverage. Schedule subject to change based on business needs. MINIMUM QUALIFICATIONS A Doctorate in Pharmacy degree and at minimum completion of a PGY1 residency program. Without residency training a minimum of 3 years of health-system progressive clinical pharmacy experience (evaluating disease state & medication management e.g. thromboembolic, diabetes, oncology, pharmacokinetics, pain management & nutrition, investigational studies, etc).
Additional minimum qualifications include board certification through the Board of Pharmaceutical Specialties (e.g. BCPS, BCOP). Board certification must be active and maintained.
For pharmacists providing care in Investigational Drug Services, the ASHP Investigational Drug Service Certificate will be allowed. Candidates who are not Board Certified will be given 24 months to achieve certification.
Requires pharmacist licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy. If not licensed in the state of practice, must have completed the Board of Pharmacy application process within 30 days of job offer. Must be licensed within 90 days of hire unless the delay is due to the State Board of Pharmacy. May be required to possess a valid driver’s license and be eligible for coverage under the company’s auto insurance policy.
Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.
A current and unrestricted Pharmacist license (not intern) is required at the time of hire for BSS applicants. May be required to possess a valid driver’s license and be eligible for coverage under the company’s auto insurance policy.
Must possess good organizational, problem solving and computer skills. Must possess good written and verbal communication skills for interfacing with all levels of staff, Physicians, and other contacts. Must be able to use reference tools to evaluate patient care and answer questions posed by customers. Contributions to the profession and organization are expected.
