Summary
The Social Worker Program Coordinator serves as the professional and supervisory leader for the Mental Health Intensive Case Management (MHICM) program - a major specialty treatment program within Behavioral Health Service. Primarily at the Veterans Recovery Center - serving Veterans in rural areas of the VASNHS catchment area. The incumbent is directly responsible for the planning - development - implementation - monitoring - and evaluation of MHICM and MHICM RANGE program operations.
Duties
Total Rewards of a Allied Health Professional Incumbent has broad program management responsibilities for the MHICM Program and has supervisory responsibilities over a large - multidisciplinary team of mostly advanced practice Social Workers and Peer Support Specialist assigned to the program
These supervisory responsibilities include personnel actions - disciplinary actions - clinical supervision - consultation with other program areas - quality improvement - and monitoring the quality and quantity of each MHICM staff's work
Incumbent works collaboratively with internal staff - VISN and VACO leadership - and external stakeholders to expedite service delivery and to ensure that the program is fully aligned with directives from VISN and VACO leadership
Duties may include but are not limited to: Program management responsibilities include the development of short- and long-range goals and plans - review and process of MHICM consults including case assignments - development and implementation of policies and procedures to ensure goal attainment - management of administrative and programmatic resources - and outcomes monitoring using data-driven quality assurance processes
Provides training - orientation - clinical supervision and/or guidance with clinical practice to the interdisciplinary MHICM program team - including licensed clinical social workers - RN case managers - psychiatrist - and peer support specialists
Develops and issues performance standards in accordance with VA policies
Coordinates interdisciplinary staffing needs and work assignments
Sets productivity targets for staff and monitors productivity
Provides leadership and guidance in carrying out MHICM program planning - outcome evaluation - and staff development activities
Participates in VISN - VHA - and other community networking activities or meetings at the behest of the PSR Chief - Deputy ACOS/BH - ACOS/BH - or Executive Leadership
Incumbent ensures that MHICM program services meet all accreditation requirements and are compliant with regulatory and accountability standards
Participates in education/CME lectures - research - specialty care programs - clinics and/or committees as assigned by the PSR Chief - Deputy ACOS/BH - ACOS/BH - or Executive Leadership
Reviews and processes all consults to the MHICM program within facility and national guidelines and determines appropriateness for program enrollment and participation in accordance with national program guidelines and program fidelity
Fully accountable for practice - including entering Veteran visits - measuring outcomes to interventions - and meeting productivity standards
Provides clinical services including direct care to 3-4 Veterans enrolled in the MHICM Program
Provides consultation from a recovery-oriented perspective to medical center staff and interdisciplinary team members on complex cases - using advanced practice skills and expertise
Facilitates patient and family education appropriate to the patient and the specific presenting mental or behavioral health issues
Incumbent - or incumbent's designee - provides consultation and assists with coordinated specialty care for veterans with early onset psychosis as part of the Early Psychosis Intervention Coordination (EPIC) team
Provides training and consultation to Behavioral Health Service - services throughout the VASNHS - and community partners in the identification and management of common and/or complex mental and behavioral health disorders
Participates fully in peer review - assessing the quality of professional social work practice standards provided in the organization
Provides orientation and coaching to new social workers and social work graduate students
May provide clinical supervision for social work licensure
