The incumbent serves as program coordinator at the GS-12 grade and is clinically and administratively responsible for the development - implementation - and management of the Veteran Directed Care Program (VDC) within the VA Eastern Colorado Healthcare System. The incumbent manages the daily operation of the program - develops policies and procedures for program operation and prepares reports and statistics for facility - VISN - and national use.

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)
Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited
A doctoral degree in social work may not be substituted for the master's degree in social work

Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level

Grade Determinations: In addition to the basic requirements for employment - the following criteria must be met when determining the grade of candidates
Social Worker (Program Coordinator) - GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level
Experience must demonstrate possession of advanced practice skills and judgment - demonstrating progressively more professional competency
Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship - or equivalent supervised professional experience
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level - and must be able to provide supervision for licensure
Demonstrated Knowledge - Skills - and Abilities: In addition to the experience above - the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation - negotiation - and monitoring
Knowledge and ability to write policies - procedures - and/or practice guidelines for the program
Ability to supervise multidisciplinary staff assigned to the program
Skill in organizing work - setting priorities - meeting multiple deadlines - and evaluating assigned program area(s)
Ability to provide training - orientation - and guidance within clinical practice

Duties
Administers information and analytical systems to evaluate and enhance the quality of service provided
Develops and implements information systems to track service activities including visits - referrals - patient demographics - equipment utilization and patient satisfaction
Performs a wide range of services and provides technical assistance to State - local - and private agencies - and universities
Technical assistance provided is based on monitoring activities and program reviews - and the development and analysis of information
Uses sound judgment in the interpretation of trends and patterns

Performs program analysis - development - review - research - design - and contact work as possible with various agencies (public - DOD - state - etc.)
Performs work related to the review - monitoring - analysis - planning - and assessment of Veteran Directed Care (VDC)
Responsible for key project activities related to the planning - development - and implementation of VDC
A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the medical center
Performs clinical duties that involve specialized treatment of complex physical or mental illness
The incumbent incorporates complex multiple causation in differential diagnosis and treatment of veteran patients - including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice
Provides individual - group - and family psychotherapy and advanced level case management intervention used in the treatment of veterans with polytrauma injuries - spinal cord injuries - traumatic brain injuries - visual impairment - post-traumatic stress disorder - addictions - and other serious disorders
Develops and implement methods for measuring effectiveness of social work practice and services in specialty areas - utilizing outcome evaluations to improved treatment services
The incumbent is able to demonstrate the ability to design changes based on empirical finding
Teaches and mentors staff and students in the special area of practices and to provide supervision for master's level interns - licensure - or specialty certifications
The incumbent provides the following functions independently: Screening/Assessment: The incumbent will screen VDC referrals to ensure appropriateness
An assessment is completed prior to entering the program with emphasis on veteran's strengths - limitations - supports systems - and service needs
Community Care referral and consultation: Incumbent will complete all community care consults for VDC and will refer to and coordinate with the agency of jurisdiction
Case management: The incumbent will collaborate with care providers and VDC staff to assist in establishing appropriate goals and make referrals based on the veteran's preference
Case management services will be provided in relation to care within the VDC program - and in conjunction with the veteran's primary care team
Provider Agreement: The incumbent is responsible for overseeing - monitoring - and tracking the program Provider Agreements - ensuring that agencies are adhering to the Provider Agreement specifications
Data and Quality Assurance: The incumbent will compile monthly and quarterly statistics on VDC
Incumbent will conduct satisfaction surveys and monitor facility records for compliance related to the VDC regulations
Incumbent will monitor and track incident reports involving veterans in the program
The incumbent may be requested to assist in the advancement of student social workers and/or newly graduated social workers for advanced practice licensure or the graduate trainee program - act as a mentor for less experienced social workers - participate or chair committees or groups that impact social work practice and/or lead roll out of evidence-based practices
The incumbent performs other duties as assigned
